Please Read Before Setting Your Appointment

If you are setting your appointment for a Live Scan Service, please make sure to bring the following: 

  • Fully filled out Request for Live Scan Service Form
  • A valid Gov’t ID (ex. U.S. Driver’s License, U.S Passport, U.S. Active Duty/Retiree/Reservist Military Identification Card)

We will not be able to process your Live Scan if you don’t have either of these items listed above. In addition if you need multiple services, please make a note under the comments instead of booking an additional appointment
